15) Name of sponsoring authority if any and plan scheme under which you came to Canada - Presently, I'm on work permit. Should i need to fill in anything here or just leave it blank?
20) Addresses where stayed: I came to Canada in Jan 2017 and haven't gone to India since then -- Shall I leave this blank? please confirm.
In the Reference form:

Period of stay in India - Should i mention total number of years e.g 30 year 5 months or need to provide with dates?
