Hi there! I have a question regarding the return of the passport of my spouse. He already submitted his passport and other documents that they need on July 18, 2013 and I'm worried till now we havent received anything from the Manila embassy.

Hope you can help me to clarify things and to clear my mind also. Thank you in advance.
 
What you're experiencing is normal. Unlike most other visa offices, Manila requests the passport at the very start of the process (rather than when they are ready to issue the visa). So again, everything is normal and all you can do is wait.
 
Do you know how long do we need to wait? By the way we started the application on January of this year.

Manila has a posted processing time of 14 months. That time starts when Manila receives the app from Mississauga, after sponsor approval. If you applied Jan 2013, I'm guessing you probably had sponsor approval Feb or March, which means you should expect to hear from Manila around June 2014, maybe a bit earlier.
 
Eventhough we already submit the passport? Do We really need to wait that long? What if his medical was already expired, did he need to have a new one again?

Yes. As scylla said above, Manila requests the passport early, so you still have quite some time to wait.

If his medical expires, CIC may require him to complete a new one. If they are ready to issue his visa just as the medical expires, they may apply to extend the previous one.
