I am trying to buy a Canada Post Shipping label to have my passport sent back to US. What is the from address I should fill in?

So far, it is working well for me!
and other than regular mail option, this is the only choice we have if we wanna track the passport!

USPS and Fedex, both needs a phone number for Return Label - for the CPCO- VIsa center ottawa , 365 Laurier Ave. west, and Icannot find PHONE number any where, what should I do, please let me know
