In the form IMM5606E Additional Family Information, what name would I'll put for my mother who got annulled then re-married.

For example her maiden name is Regina Park then when she got married it becomes Regina Hudson and when her marriage got annulled and re-married it becomes Regina Thomson. What name name should I use for the application?

For my father's name my biological father or my mother's current husband?
