I have got below inquiry before few days.

Proof of Relationship (SPR): As proof of relationship to the principal applicant, please provide your voter ID card, Aadhaar card and old school records (Fifth class, Middle standard and Matriculation/secondary school certificate). If you are unable to provide these items, you may provide the principal applicant's Ration card, your Indian passports (expired and current) and/or OCI, and PAN/Income Tax card. NOTE: Affidavits are not acceptable as proof of relationship. If you are sponsoring your grandparent, you must include a copy of your mother or father's birth certificate, as well as your own birth certificate in order to support your relationship to the principal applicant. If the document originally issued by a government authority was not in English or French, please provide a notarized (certified) copy of both the document in the original language [birth/marriage/divorce/death certificate] along with a certified translation in English or French. In addition also include as proof of relationship to the principal applicant, please provide your mother's or father's voter ID card, Aadhaar card and old school records (Fifth class, Middle standard and Matriculation/secondary school certificate). Questions:
1) My PAN card has different name of father. for example, instead of "first name kumar" it says, "first name bhai". should i send PAN card or not?
2) My father does not have ration card.
3) Since i am citizen of canada, i dont have voter ID of india, I am sending OCI and cancelled indian passport and both shows my father name correctly.
4) I have adhar card but it has my husband name.
5) I have higher school certificate, school leaving certificate and bachelor's degree. it has my full name where middle name shows as my father name.. but there is no particular line which tell me father full name. so should it be okay?
4) should i write any explanation Letter? if yes, please help what to mention.

This is pretty common , you should send what you have ... if first name and last names are switched its still good enough but send all other things where your name and their name is written. Worst case scenario IRCC accept the DNA result too .
