HI EVERYONE,IM A LIVE IN CAREGIVER FOR ALMOST 4 YEARS NOW & I DIDNT APPLY FOR AN OPEN WORK PERMIT YET & PR & MY CONTRACT WILL BE FINISH ON SEPTEMBER 2014 & MY WORK PERMIT WILL BE ON NOV. 2014,.. AM I STILL ELIGIBLE TO APPLY FOR OWP?

Yup,you are eligible to apply before your work permit expires...
Complete all the requirements and you are good to submit your application for owp and pr under lcp.anyway,why u did not submit it earlier?

Is IMM 5710 is the right form for application of work permit?
coz I have a big question about completing the form,..in Question#3

Its says here: Check the appropriate box or boxes to indicate if you re applying for:
. An extension of a work permit with the same employer
. A work permit with a new employer
. Restoration of your status as a worker
. An initial or an extension of a temporary resident permit

so which one pls?

Yup.that is the right form.just write on top:Open Work Permit
Just check the 1st one since you are staying with your employer till september.submit your application asap because it takes 3-4 months for the processing of owp.if u submit it this month,maybe by the end of august you're going to receive your owp...

Can I still indicate " AN EXTENSION OF WORK PERMIT WITH THE SAME EMPLOYER " ??? Though they're not going to renew my contract anymore?
