I am applying via Family class for Spousal Sponsorship. I have my checklist and all my forms together and I'm going through everything and starting to fill out my form. I'm applying to be with my husband (In Calgary) From OUTSIDE of Canada- I live in Washington state. We have just recently been married this year and I'm still in the process of switching my name over on my documents here. Pardon me if my questions are a bit stupid, I'm new to all of this, and I want everything in the slightest to be correct to be sure there are no mistakes or issues. :D
My questions are:

[size=10pt][size=10pt][size=10pt]1) IMM 0008 pg. 1: "Visa Office Requested for the process of your application: " ? Do I put the office I'll be sending my application to? Because I didn't request it, I printed out the whole packaged form + guide.

2) IMM 0008 pg.1 q.1: Under "Your Full Name" Should I put my now married name, or my maiden name? It says as shown on Passport or Travel Document, should I have my last name changed to my married name on my U.S passport BEFORE applying? So far my I.D., Social Security, Bank accounts, etc. Has been switched over to my Married last name.


3) IMM 0008 pg.1 q.12: I checked "Non-university certificate/diploma" As my highest level of Education, because I am a Certified Nurses Aid, and I have the certificate. I did complete secondary school as well, and I know I can't check more than one, so that's fine, correct? Will I need to send in a copy of the certificate/high school diploma?

4) IMM 0008 pg.2: DETAILS OF FAMILY MEMBERS- My husband and I are the only ones applying, we have no children, nor family members accompanying me. Can I just fill out the information for me and under Relationship to You put SELF? Do I also need to fill out information of my Husband as well? And again, Do I need to use my maiden name or my married last name? (Sorry, so confused by it. I over think everything.)

5)IMM 008 BACKGROUND DECLARATION- I don't know whether to check Principal Applicant or the box that says the spouse of the principal applicant. Also- once more, Family name, my printed full name, and Declaration signature, do I use my Maiden or married name? In general through out the application, I sign with my married name, yes? (ugh.)

Every document I have now is switched to my MARRIED last name. When signing any forms I sign in my married name, correct?
Also, is there any steps I should be taking now while filling out my application? (Fingerprints started, pictures taken, etc.) My husband and I aren't worried about time, we just want everything to be legitimate. Thank you so much, these are minuscule questions, any little help goes a long way, I'm a bit overwhelmed!

For the office to process, it's Buffalo for a an outside-Canada U.S. application.

You do need to use the name that is on your passport. Somewhere, there is a place to put an alternate name.

As far as I know, they don't want a copy of your nursing certificate, just to know that you have one. I don't think this is critical for a spousal application anyway, but just a standard question.

IMM 0008 pg.2: You don't put yourself in there, only your spouse (if he's not Canadian) and your dependent children (if you had any). Just put "not applicable".

If you are a U.S. citizen, you are the principal applicant. If your husband is Canadian, he is your sponsor. You are not bringing a spouse with you unless you, for example, are being sponsored by your parents or some other eligible family member. You haven't mentioned any other family members, so I assume that your husband is Canadian (or a Canadian PR).
 
for outbound us citizen applying for canadian PR for visa office requested i had put CPC Mississauga, ON L5A 4N6...so from what I just read i should change it to buffalo?
 
Yes - it should be Buffalo (not Mississauga).
 
You send it to Mississauga but they only process the sponsorship. The application itself is then sent to Buffalo from Mississauga for processing.
