Just curious to know this point.

So in the relationship evaluation form, there is question asking for other sponsorships done by the sponsor before.

Written in the guide: You must include everyone you have ever sponsored, even if they have not become permanent residence of Canada yet.

Ques: What if the sponsor has once submitted a PR application but it was sent back due to errors in it?

I don't think you have to mention the person there, but you should include a letter explaining the previous attempt and that it was returned.

When you say a letter, is it just a letter written and signed by me or a notarized one like an affidavit?
 
In the OP-2 CIC guide (and IP-8) for officers, an application does not exist until CIC has:
- Payment
- Correctly filled out IMM 1344 and IMM 0008, and Schedule A IMM 5669.

If they returned the application because of errors, it didn't exist unless you resubmitted with corrections and they started processing.

So you wouldn't have to mention it.

5.4 When does an application exist?
An application in the spouse or common-law partner in Canada class requires receipt by the CPCM of a properly completed and signed Application to Sponsor, Sponsorship Agreement and Undertaking [IMM 1344E], a properly completed and signed Generic Application Form for Canada [IMM 0008E] including the Schedule A Background/Declaration [IMM 5669E] and proof of payment of the correct processing fees. Under the spousal policy, H&C applications with a spousal connection will be considered applications in the spouse or common-law partner in Canada class after the receipt of a sponsorship, if not already submitted. For further information see:  Sponsorship application and minimum requirements, IP 2, Section 5.12; and  Minimum requirements for applications, IP 2, Section 5.13.
