If the form was updated before you sent in your application then it could result in getting your application returned, but it might not. You might get asked for an updated checklist and the NOA instead of the Option C, but you might not. I wouldn't worry too much about this one. There's not much of a way to tell until you get AOR.
 
I sentthem the both option c printout and notice of assessment and i let them to choose the required document

Hi guys! I was wondering how long it would take for us to know if we lack a certain document? I feel worried because I failed to send the last page of my Option C print out (though it contains only a few lines, good thing the the total and net income happened to be on the first 2 pages which I have submitted). I sent my application mid February and I used the old checklist where option C was required that's why I'm a bit upset since I did not noticed that there's a new checklist as we already completed the checklist end of January. :(
