Open work permit-spouse of full-time student in UWO, urgent!

My wife studied full-time in UWO as a post-graduated student, and i wanted to apply for an open work permit.
I got 6 months trv and i arrived in Canada on Feb,4 2013.
The people in London public library told me to choose the 4th option.
1.A work permit with same employer,
2.A work permit with new employer,
3.Restoration of temporary resident status as a worker
4.initial temporary resident permit or extension of temp resident permit.
But i am now applying for an open work permit.
Is that right?

Is there anyone can give me some help? thanks!!

Thanks for your advice.
Did u apply this kind of visa recently?
It's changed a lot since maybe last December.
Is there anyone else who has applied OWP recently?
