Open work permit - IMM5710E queries

Hello,

I am currently on Temporary Resident Visa & in process of applying online application for Open work permit from Canada.
Can someone please advise on how to add additional information for 'Employment' section in form imm5710e to include last 10 years of history?
If I put this additional information on a separate page, do I need to combine this additional page with copy of imm5710e or can I upload this additional page separately like imm5710e?
