Hi,
I had called and asked OOPNP about this. They said you may attach 10th Marksheet + Certificate of birth (you can get this document from Indian Consulate in Canada).
On a side note, check with the Registrar of Births and Deaths at your place of birth, they may be able to make one for you in 2 weeks or so.
Also, please understand that 'Affidavit' or 'Declaration' ONLY means that you are under oath, and whatever you write, if it is not truthful, you will be committing perjury. So, literally speaking, an affidavit/declaration is only as good as your word, it is not a proof of anything. Also notarization simply means that you signed the affidavit in good conscience and the lawyer/notary made sure you signed it in front of him (you may even write 'I have wings to fly', and the notary can notarize that as long as you say to him that it is what you in fact believe).
Hope this helps!

Thanks for the reply. Yes i understand Affidavit' or 'Declaration' ONLY means only oath. But the same affidavit has been submitted by few of my friends for applying for PR under CEC last year and it has been accepted. Can't we use the same for OOPNP nomination??

Also how many days it will take to get 'Certificate of birth' document from Indian Consulate in Canada.

Thanks and Regds
