Hi fellows just wanted to confirm, that there is officially no documents attestation requirements "notarised" or "justice of peace" in the checklist form. I know we can attest voluntarily if we want, which is different story, and I don't believe there is any harm in this. I meant there shouldn't be any issues if we provide plain copies e.g. Passport, civil status documents, bank statement, home lease document,date of birth, utility bills etc etc. isn't there any form that need to be evidenced by notary or justice of peace, but I couldn't track any? Please correct me if I am wrong. Good luck to all

Hi kabayan ymv, i have several questions:
- Which application form did you place the red sticker?
- Did you have all the supporting photocopied documents (i.e. passport, birth certificate, ielts, etc.) notarized?
- I just got my NBI clearance and the comment on my clearance says "No record on file." Did you get the same comment?

Yes that should be enough.. most notaries just attach a page on top and mention no of pages.. so you should be good. I did same thing in my case

for getting PCC of U.S.. details below.. need to go to local police station and get ur finger prints.

Hi pinay2005,

UCI - is the Universal Client Id found on your visitor/work record issued when you land in canada. Its an 8 digit number xxxx - xxxx called Client ID. Leave it blank if you dont have one.

The Background declaration from IMM 5669 and the additional family information form IMM 5406 need Dependant Signatures, assuming that the Dependant is a spouse/child over 18 years of age.

Thanks Kia. I checked both forms but I do not see any sections where it says dependent signatues. Everywhere it is asking for information and my signatures on the confirmation. If possible can you point to specific page and section?

I am new in this forum as i have nomination for alberta, but as i have a common query i thought someone may be able to answer me here ,tell me how to fill IMM 5406 for my spouse separately, how it will be mentioned that details belongs to the family of my spouse. plz advice.

Hi khan, just let your spouse fill up the first line which says applicant. The form doesnt say principal applicant.

Yup, you're right not unless your documents are not in english. That's the only instance when you'll need to have it notarized.

Hi, I placed "Federal Skilled Worker" since the form says "Economic Classes - Provincial Nominee". It'll be redundant if we place "Provincial Nominee". Besides, it says also on my nomination certificate "Federal Skilled Worker".
 
Again, I may be missing something but I do not see any place where dependent signatures are required.

Your dependent (assuming its your spouse and/or child over 18 years) has to fill out both IMM 5669 and IMM 5406 and sign it.
