I think you should fill WP form for your spouse and TRV form for your child (there should be a minor TRV form for children) , and I don't know about Good Conduct, it may be PCC.

As I expected My WP application was returned to me due to further inquiries

They asked my to add the barcode application for my spouse and kids , does that mean the WP application IMM 1295 ? and if so , what data should I fill for my 2,4 years old kidds ,so confusing

Please if you have any idea about my questions please share

Barcode application for ur spouse and kids are the TRV forms. Filling "IMM5257B_1-Schedule 1 Application for TRV" is not enough.
There is "IMM5257E-Application for TRV" Its a 4 page form with Validate button. At the top it says "APPLICATION FOR TEMPORARY RESIDENT VISA"
and not" SCHEDULE 1 APPLICATION FOR TEMPORARY RESIDENT VISA"

We suppose apply for Open Work Permit, however everywhere its mention for Work Permit, for which we usually required Offer letter etc.

From 15 Dec 2012, we can apply online, even there is no open to select Open Work Permit, due to which its asking for below documents and we cant proceed until we attach given document.

So anyone has applied online for Open Work permit? if yes then which open has been select. I will appreciate to get the response as earliest.

Folks, today I received my medical request by email. The email body is carrying the CIO-Sydney office address. I am still not sure where is my visa office. How do I find out where my file is? I am waiting to find that so that I can send my PCCs to that address

if you have PCC on hand, dont mail them now but wait until you are asked to mail them.
If you just want to know your visa office (and you are in Canada), you cal call CIC phone center 1-888-242‑2100.
But it may take time till the file is actually forwarded to the visa office (in my case its still not forwarded after 1.5 months)

My problem is that my PCC's will expire next month. They have a 3 month validity. Have you sent yours? Also where did you send your Medicals, which address CIO Sydney or CPP - Ottawa?

I provided the address of CIO-Sydney to send the PCC. As I understand, they will forward it to the visa office in the worst case.
Regarding the medicals, our doctor said that medical results of in-Canada applicants are sent to Ottawa medical center, and from there they are automatically forwarded to the visa office (it may take about 3 months).

Ok, so you sent the PCC's later as a separate package? Also did you affix the red sticker on the package with the PCC's? Do you need to give the red sticker to the doctor for the medicals? One last question, the photographs you have to give with the medical form, does it have to be against a white background? Sorry for so many questions and thanks once again.

I think it's better to attach red stickers to any correspondence with immigration services. I personally didn't attach them because I forgot .
My PCCs were sent by the consulate in a separate package.
As I know, photographs for the medicals should be done in the last 6 months and can be at any passport format, because the format isn't specified in MR. I made them in Canadian passport format.

I received AOR from CIO yesterday. Please update tracking sheet (Payment realized on 7 Dec 2012). Thanks and wish everyone else good luck.

Also, can anyone clarify about the next stage and expected time? Is it AOR from VO or MR? I still need to submit my UK PCC so want to know when I should do that (Someone told me that I should only send it out upon receiving document request from VO, what stage will that happen?). Thanks.

I got it by email. I think you should get it anytime now. No it does not mention any processing duration, just your Client ID and File number. No Mention of medicals either, it comes as a separate email after about a week.

unfortunately mine was returned because I didn't include WP form for my spouse, my VO needs 3 form for each person :WP or Temporary visa form for each, Schedule 1 form for each and Family information for each, so I should send my package again.
