Online receipt pls help?

Hi guys, I just made payment for the sponsorship application online, and there is a section to be filled on it. Pls who name is filled, the sponsors name or the applicants name and date birth. Pls help me guys I need to send this off ASAP !

If you made the payment with your credit card, then use your info. I don't think it matters whose name. In my case, my sponsor made the payment and wrote down his info.
Leave the Client Number blank as you probably don't have one (unless you do).

It's supposed to be the sponsor's name.
