i paid my online application i printed receipt and i am confused where it ask to put name and client date of birth? should i put my name or my sponsor name? REALLY URGENT!

We put the sponsor's name on it and info. It worked fine for us.

Yes it is the sponsor's info....took my a while to find that instruction when I did it...but I certainly won't foget!
