Do you have to live together for a complete year before you can apply spousal? I know this is true for common law.

There is no time limit for married couples. You can marry today and apply tomorrow.

Just be sure to keep that visitor visa current, but yes, should be able to.

No, but she should have her marriage certificate and inform them she would like to 'visit' with you so you guys can file a PR application. They will give her a visitor record and attach it to her passport. It may only be for a few months initially, and she can then renew that online, or if you are close to a border then at the border. They will likely only give her a maximum of like 6 months to get the PR application started. She should NOT bring a bunch of her stuff with her. If you are crossing with her, it will be that much easier.
 
They only gave me one for 3 months initially. Once my application was filed, I brought them back that proof, with payment receipts, showed them my police clearances that said I was a 'good girl' and they gave me a 12 month record. I didn't need to renew the 2nd one.
 
What if we married in Canada and send all our paperwork outland? still need a visitors visa? and I know she would have to get her fingerprints done in the U.S. but could she do her medical done here in Canada ?

Yes, would still need the visitor record to keep a valid 'status' here in Canada. You don't want potential hassle of CBSA tracking her down to deport her. Not worth it, so keep the record and keep it current. She can actually get her fingerprints done in Canada and then send the card to the FBI from here. They will also mail her response to a Canadian address with no problems. Yes, she can get her medical done in Canada, it only needs to be done by a CIC approved panel physician. You can find a list of those in your area on the CIC website.

If she is not YET married, then all she needs to tell them at the border is that she is here to visit her fiance for a few weeks/months. However long she will be initially staying. If they stamp nothing in her passport, she will have 6 months. If they stamp something in her passport, she has until that date, so it will be important for her to renew that status before it expires.

I don't know if she is planning to go back to the US at all after the wedding or what you exact plans are. The important thing here is that she keep her status while you guys get things together. It is going to take you roughly 3-4 months to get EVERYTHING together for her application to be sent in.
 
She probably will be going back to the U.S. once or twice but i think she is allowed to for up to 3 weeks at a time. Even though it will only be for a few days to visit family .
I kind of figured it would take ahile to get things together like the fingerprints and medical , then send everything in at once correct?

Ideally, yes, send it all at once so you don't have to worry about it not being there, or trying to match it up to the application later on.
