1. Do middle name columns also have to be marked N/A ?

2. In the mailing address I have checked "same as above" -do I need to write anything N/A here or left blank after checking "same as above".

3. Also for where do you write House number in address details in No suffix or Unit,suite,Apartment. eg Indian address is 78B

4.In the document checklist section there is a field that says
"evidence of good academic standing and length of programs completed (spouse or common law partner only)

Do you have to check it or leave it blank (my wife has never studied in Canada but I am sending her copy of ECA report)

write N/A and submit your wife ECA with your application and a LOE highlighting it.

Should you really submit your wife's ECA report? The evidence of good standing for spouses only applies to Canadian education. I'm filling mine out now and I was not planning on sending in her ECA report, this is because for FSW the spouse's education level is not assessed.
