OHIP Application Requirement

I'm planning to apply for OHIP in July. Currently I'm living with my friend, as a result I don't have any rent or lease agreement under my name. I'm planning to get bank account statements to be delivered to my friend's address, where I'm staying and using the statements as proof of residency.

My question is, how many months bank statement is required for proof of residency?

Probably one. Will you have a lease soon? Your ohip card should have your permanent address so if you plan on moving soon you should wait until you have that address.
