Well to answer the question of what you should submit, you need to fix the mistake on your spouses application form then return it back to IRCC for processing. They will return the entire application pack to you, this will include your application and that of your spouses. Same happened to us and we just fixed it and returned it to the address indicated on the letter which will be included.

Do you fill all the application form again? In my case I don't have any issues with the application form; They returned it as I have submitted the academic IELTS.

I think you need to make sure that the signatures are not older than 90 days (I believe) on your application. If you can meet that, I think submitting an IELTS general scorecard alongwith the older application should be acceptable. However, I always play very safe. I would've submitted a completely new application alongwith probably the same fee invoice
