Hello,

I was wondering if forms such as "Use of Representative IMM 5476" and "Additional Dependents IMM 0008DEP" are meant to be included in the application even if they don't apply to you?

Do you include them and leave them blank or write N/A at the top? Or leave them out all together?

If the forms aren't required, you don't need to include them. You should leave them out.
