Is Pathology Professor a restricted profession in NOC 4121? Is MD/Ph.D from USA acceptable for immigration purposes in NOC 4121 or there is any restriction? It is not mentioned anywhere. For job purposes, I know
CanadianBoard exam has to be passed. I do not know about immigration. Any comment?

You have to separate the immigration requirements from job requirements.
For example, as a psychologist who has been a university professor, I'm in a similar situation.
My PhD and work experience qualifies me for immigration (I hope - still waiting for my letter from Sydney).
Registration with the provincial college of psychologists, however, is required for me to practice in Canada.
Job postings for a psychologist, however, may require that I'm registered with the provincial college of psychologists. And, some university teaching positions, such as in a graduate training program, may also require I'm registered. Of course, other positions may not.
Take one step at a time. Apply for and get your PR visa. You may find you will need to pass some type of board exam, etc. If you need to, you may need to work in a lab or teach outside of a medical school, etc. in the meantime. A friend of mine (who was an MD in Mexico) worked in a research lab in the US, for example.
