I think you should receive PPR by June, as your new medical request shows everything is done and they need valid medical exam with 6 months validity to issue visa.
So from now on you should expect it every moment.

Dear Nima

Once IRCC issues the eCOPR, the applicant has one year from the date of issuance to enter Canada. Failure to do so within this timeframe will result in the loss of immigration status. During this period, the applicant must enter Canada with a valid medical examination, regardless of its specific validity duration.

Submission :- Sept 2021

PAL :- Feb 2024

The last request was in October 2024 (6 months ago) requested Schedule A and travel history for my sons "twins" because they turned 18 years old in Sept 2024.

That's such a funny coincidence, my twin girls turned 18 in October 2024, so we also got the same request in October.
