No surname while registering for PGP 2020 sponsorship

Hi All,

While filling out the PGP sponsorship form I had noticed that my parents don't have a SURNAME in their passports. Neither do I also have a SURNAME on my passport. Just the GIVEN NAME section is filled. There are 2 sections while filling the online application form:-

1) Family name(s) (also known as last name, surname)
2) Given name(s) (also known as first name and middle name)

Also under the Given name section they have mentioned "By checking this box, I confirm that I don't have a given name (first name)". So should I just fill the surname and check this box? Or would you suggest me to fill in both SURNAME and GIVEN NAME with the same information?
