NIC translation and its true certified copy (Pakistan)

Hi,
anyone have recently done translation of NIC Urdu into English from Pakistan?
I need to get the contents sample of Affidavit and True certified copy, as here in Hyderabad Pakistan notary doesnt even know what to write on stamp paper and True certified copy page.

Please help
