Hey guys,

A quick question on the reassessment for the medicals. In the email it says submit REPORT CHEST X-RAY REQUISITION AND REPORT form IMM5419 page 5/6 but also Duly completed Re-assessment Form is there a second form to complete?

Any one who has done the re-medicals have an idea?

1.passport and copies of passport
2.2 photographs
3.the e mail that's been send by medical immigration officer
4. The attachment which was been attached in the e mail

The rest of the form filling procedures immigration officer will take care don't 4get to have your uci no handy as that's needed

Hi,
When you are mailing additional documents to ndvo you have two options:
1. Via courier ([remove the po box no]which will be collected by the receptionist you will get an acknowledgment)
High Commission of Canada
Immigration Section,

2. Via post (where the postman will deliver it in post box)
High Commission of Canada

We didn't get any attachment in the email but was able to find the IMM5419 form from the CIC website for which page 5/6 has to be filled but they are also asking for a Re-assessment Form any idea on that?

Also it says we have to fill page 5-6 that is correct right? what is the rest of the form filling the officer will do?

We didn't get any attachment in the email but was able to find the IMM5419 form from the CIC website for which page 5/6 has to be filled but they are also asking for a Re-assessment Form any idea on that?

Also it says we have to fill page 5-6 that is correct right? what is the rest of the form filling the officer will do?

You no need to take that IMM5419 form..that will be done by panel physician only..
they will send all the required forms to delhimedical
i can send you that reassessment form if you need..

So much confusion so there is only one form and we don't have to fill it - I just read the form it asks specifically for the them to fill it.

We no need to fill any form.... take the re-medical requested email, passport size photos, reassessment form and passport to the hospital they will do the rest....
