my office is scarborough

i left canada for 1 month

i need to get my health record i mean my visits to the doctor and so on

Go here and click on "Request for Access to Personal Claims History Information": http://www.health.gov.on.ca/en/public/forms/les_fm.aspx

Fill out the form, print it, and mail it in. It reportedly takes about a month.
