Hello Experts,

I cant fill all my personal information in the mentioned Personal history column of SCHEDULE A form and it is clearly mentioned that i need to attach extra sheet to provide extra information. Can anyone please share the extra sheet format that would be so great thanks

Simply make the boxes in a word document. Columns and rows. At the top of the extra sheet mention that it's attached to Schedule A form.
This is what I did.
 
I was wondering if we can just take extra print outs of that form and attach as extra sheets with additional information in it.
