need some help from you guys

i got my nomination from sinp sep 2014 then i applied PR under provincial category ,

month of feb 2015 i got my file no and medical. it looks my pr will granted soon .

but my work permit will be expire end of December 2015. if i don't receive my PR before December 2015 then i have to extend my work permit. As i have letter from sinp called work permit supporting letter it means i can extend my work permit without LMIA but from 21 february 2015 there is new law that employer have to fill IMM5802E and have to pay $250.

my question :
where my employer have to send the form which address ,
if my work permit expire December 2015 when is the ideal time to send the form,
is there any approve or declined issue about this form or this this just formality
