Hi Everyone,

Greetings!
My mom is identified by two names. Her name in my birth certificate is different from the one appearing in her documents.
Can anyone please suggest me which name to provide while filling family details for my permanent resident profile?
I am planning to get an affidavit stating she is recognized by both names. Will this be sufficient while filling permanent resident profile?

An affidavit called "One and the same person" you should look around this forum, i'm sure you'll be able to find a template.

Besides the affidavit you can write a letter of explanation regarding her names.
