in the generic form it asks "how many family members are included in the form, including you", i have two kids, one is a Canadian. so should i write 4 or five "including my Canadian daughter ".

5 should be the answer since the question is about the number of family members you have. By the way, if you have two kids then how come you have 5 family members?
