Need help for PR form pls..

Hi everyone! Iam am new in this forum this was recommended by my friend regarding my confusion regarding the PNP n CIC PR form..
This is the question. I delare separated from my husband. I saw some form in PNP and CIC form that there are some forms that need to filled up by spouse. If I declare separated does this form applicable to me or just ignore this form? If ever He need to sign it the problem is I dont know where he is because we dont have communication anymore coz he has new family now. Is the visa office will consider this? I have the custody of my child so I will only put my child on my application form and as dependant? Pls give advice so I could finish the form! Thanks a lot I am hoping To get a lot of advice...
