Names are different in IT returns - FSWP 2014

Hi All,

I am planning to apply for FSWP 2014. I have the following query. Dear Seniors, Please clarify.

My name is mentioned in the passport fully: Naga Venkata Surya Kumar Srineedi

But in educational and employment letters it was shortened to NVS Kumar Srineedi. This is fine.

I am submitted last 5 years of my tax returns as proof of employment and in the 2 years tax returns it was mentioned in the following way.

Kumar NVS Srineedi and in another one N V S Kumar Srineedi

Since these returns are prepared by Tax Return preparer, he entered firstname and last name differently.

PAN is same in all the returns and everything is correct.

Would this cause any problem for my application /Am i Good? Do i need to send any affidavit saying that all are my names only ?

Submit a sworn affidavit stating all the above names and that it belongs to the same person. Also use a covering letter to explain the situation.

If I submit affidavit , do I need to consider that as a name change in the application forms and click "Yes" ? Like in IMM0008 and other forms.

No. The name was never 'changed'.

Is it compulsory to provide tax returns ? I dont see them listed in checklist and at the moment I work in a tax free zone , earlier I was working in my home country where income tax did apply but the company was lazy with this and they used to tell me that they deduct income tax however I dont have any documentation on this as of now .
