Name different in Birth certificate - what do I need for proper PR application ?

Hi,

In my birth certificate, my name is shown as X. But that name is never used anywhere. In all my documents, it is mentioned as Y. My birth certificate doesn't have the full name of both my father and mother. They are written in firstname.initial format.

My 10th and 12th school mark sheets don't have my father's name as well.
I used a sworn affidavit for name change and got my passport with name Y and the passport has both mother and father's full name.

Currently, I'm in Canada preparing my PR application (CEC). I can get a copy of the birth certificate from my city's corporation website (Chennai) online, but my name in that birth certificate is different than my name in passport, what supporting documents would I need to support my PR application ? Or how to correct this name issue?

You don't need your birth certificate. A birth certificate is only required for dependant children.

there are two ways
1. provide affidavit to corporation in your birth place city and change name on it
2. give change name affidavit as you given while preparing your passport as sometime they check your passport history as well
No. 2 is best as you already did it while making your passport
