Situation:
My family name was missing on my original birth certificate since the beginning.
Around the age of 18, I had my family name added officially to my lone first name one of my previous passports

Q1: Do you call the above "change of name" in the first place?

Q2: If so, then do I need to submit a copy of the page from that previous passport as evidence of addition of family name?

is there a way to connect that the two passport belongs to the same person. For example, when my passport expires and i get a new one, they will put my old passport number on the new one on a separate page. If that is there, then CIC can see that it is the passport of the same person.

You can call it change of name, but in most countries, for change of names or addition of names, one has to do it formally through a formal process. Did you do that?

- Yes I did go thru the formal process, and that is how eventually my passport was stamped+signed officially by my embassy.
- And yes, just like your suggestion, my passports are linked to each other, every time the previous passport stating that a new passport of number ### has been issued to this person.

So what's your opinion now?
 
send documentation from the name change process and your bc and new passport. Thats enough.
You could also send your old passport biopage and your new passport page showing your old passport number as additional proof.

Send them proof that is logical and that they could follow. I read your post again and you said, it was added to your old passport. Since the last name was added, there is no way for them to see that it was added after the passport was issued. (at least thats how it sounds to me).

Main thing is to send BC, you passport biopage (new passport) and your document form name change process. Also a letter to explain. EXPLAIN, DONT CONFUSE.
