Name Change on Passport

My passport is due for renewal next month and I will be applying at BLSIndia Vancouver.

I changed my name back in year 2000 and got another passport with the new name.

Do I need to mention the name change again for my new passport?

Depends on how the question is worded.
If it says "Have you changed your name/address since last passport was issued", then NO.
If it says "Were you known by any other name ever"., then Yes.
