Need help regarding affidavit that needs to be submitted while applying for name change in Indian Passport (after legal name change in Canada)

Can this affidavit be notarized by a lawyer in Canada? How should I go about it? Anybody who has done it please advise.

Any notary public in Canada can sign this affidavit. You can simply print the affidavit and carry your identity documents for verification.

While applying for OCI application, there is an affidavit required for name change (Indian passport has different name & Canadian passport has different) from lawyer. Could someone share the link for format of name change affidavit.

Even having a Name change cert. from Canadian govt. is this affidavit required.
