We have to put name, date of birth and address of ex partner, but completely forget date of birth, period of relationship and have no idea current address. It has been a long time ago. We certainly don't want to contact the person and ask about the dates of course. Anyone have any experiences in this?

Put the best information you have and include a letter of explanation stating which information you are uncertain about. They can contact you if they think it's critical info.
But: if you were married to the person, the marriage and divorce certificates should include this and they will ask for it. If it was a common law relationship and there were no children, they quite possibly may decide not needed.
