myCIC checklist - Form IMM5475 - HELP

Hello all, my other half is applying for extension of visitor status and his myCIC checklist is requiring form IMM5475 "Authority to Release Personal Info to Designated Individual"....problem is we can't seem to open the attachment and I can't find it on the downloadable application package. Is this form hiding somewhere else? He checklist is also requesting IMM5476 and I was able to pull the "Use of a Representative" form from the downloadable forms, but am having troubles with this other one.

Can anyone offer some help or guidance with this?
