My wife doesn't have a father listed on her birth certificate. She knows his name, and has spoken to him once, but has otherwise been estranged from him her whole life.

Is that enough to put "Father unknown" on the relevant questions on the application?

She knows his name, so that would be a lie. She needs to put the information she knows and explain on a separate sheet.
