The question clearly asks if you have ever been refused a visa to any country.

Your sister needs to respond and say that she answered incorrectly. She needs to provide a list of all previous refusals to any country. She should apologize for answering the question incorrectly and hope she does not receive a misrepresentation ban.
 
She used her phone and thought that question is only to an ETA
She doesn’t speak English she was using Google help for it to apply

The reason doesn't matter. It was her responsibility to answer this question correctly.

She needs to respond to the email she has received and explain why she did not declare the refusals.

No - not everyone is banned. Some people receive the 5 year ban and some people do not.

How many refusals did she fail to declare?
 
Her ETA got approved right after submitting the requested letter explaining that my sister made an accidental mistake or un attention
