Hi all

My mother changed her name completely after marriage .. e.g. from Sunita Kumari to
Vaneeta Chohan... so now in the Schedule 1 - Background declaration...question 8 - personal details of mother
what should be her give name and family name at birth?

Her passport shows her give name as Vaneeta Chohan... and surname as empty...

should i just put it this way
family name at birth- empty
given name- Vaneeta Chohan...


OR

family name at birth- kumari
given name- Vaneeta Chohan...

where should i mention here maiden name? or should i mention it at all ?
 
You can mention that in a cover letter. You need to show some proof of the name change like birth certificate with original name and may be a marriage certificate with the changed name. then you are done i guess. also an affidavit of some sort saying that both persons are same may help.

i dont think i would need to show any affidavit of any sort... i think that would only be required if I file for my mothers PR under family category after/when i get the PR under FSW....

Seniors pls reply....I have to send the application in 2 days

Hi,

Am in the same position as you were. My parents both had there names changed and they are both deceased now. I will have to send my BC TO CIC for my spouse application and the will see parents name on my BC is different to their name at time of death that I have put on the application. Did u have to send proof of any kind please?
