missing appendix a form to submit along with passport

Hello team.

My husband (in bangladesh) has finally received an email from singapore to submit his passport along with 4 pictures, proof of perminent residence and appendix a (a form that needs to be filled out). However the form is missing, is not included in the attached that they sent. It prints as a blank sheet. We have already emailed singapore regarding the missing form but who knows how long it will take them to respond. My husband is to send all his documents ASAP since his medical clearance will expire in march 2014.

Would someone know where to find Appendix A form???

Are you referring to the Country Specific Guidelines for Bangladesh?

Hi Tanni, As I know you will get your Appendix A form only from visa office along with your PPR. That form is not in the net. Your husband has to fill that form and send that to visa office with the passport and other requested docs. May be they forgot to attached it.

Wait, are you able to see an attachment and you found it is blank form? If yes, may be you have old acrobat? can you please check your acrobat version? if it is old version then download and install the latest version of acrobat and you will be fine.

No, as I know that Appendix A is not the Country Specific Guidelines for Bangladesh. May be I am wrong.

umm..no i am able to open the file...everything shows expect that particular page...the logo on the page shows up too but not rest of the info..unsure why...

soo stressed...now hoping that singapore will respond in a day or two

I did but it still doesn't work. I have tried on two other computers and still same.

just for an update,

I have heard back from immigration section in singapore..they emailed the correct forms.

We didn't catch another mistake that they made which they corrected in the new forms and that is they have sent us the wrong instructions for sending the passport. The first instructions have said "Instructions for the residence of Malaysia". And the the new instructions says: "Instruction for the residence of Bangladesh" which is correct.

I cant believe how careless they are. Here we wait months after months and they don't seem to give a damn about it.
