Yes, she is shown as a dependent on the IMM 008.

But I want to inquire that spouse and dependent children’s also have to fill additional family information form and schedule A form. But my daughter is only 5 month old, do I also have to fill her separate forms

No your daughter doesn't need those forms. Only dependent children who are already married or are over age 18.
