Minor Surrender Certificate Docs

Hi All,

BLS seems busy, waiting for 35 mins in queue and agent transferred me to other queue where call got disconnected. Pathetic service by them.

Can I know what documents are required for submitting Child's Passport surrender application? Child can Sign the application.

Also who should sign the checklist & photo copies, whether the child or both parents?

The first page of the application form must be signed or have the name printed of the child if the child is over 5 years old. The second page of the application form must be signed by either parent.

The following copies must be signed by both parents:

• The child’s passport (from the country of issuance) and Canadian passport
• Proof of address (parents)
• Citizenship certificate

A photograph (35 mm × 45 mm) must be uploaded with the application, and one physical copy of the same photo should also be submitted at BLS center

Mine application was online, and I have uploaded and submitted my application.
Anyone who is reading this, please uploaded all the docs online also, else they will put the application on hold.
