MI-1 Pakistani applicants medical completed, waiting for PPR1 - PPR2

hi,
i have recieved passport request step 1 from london visa office in which they ask me to send updated information chart.
my question is do i have to send only one information chart which includes other family members information or should i send one for each member of family.

Only one Information Chart is suffice.

Dear Friends

I send my initial application on 20 Aug 2009. My NOC code is 0213. My Medical was done on 10 April 2012. Some additional travel information information was asked from me alogn with medical which I send to CIC London and it reached to them on 24 May 2012. Since then I havent heard back from them. My Visa Officer is RIH.

Need guidance from senior fellow members how to approach CIC and what should I ask. As I am following forum members and most of them have received PPR1 and I am still waiting for it.

my medical was done on 31st october n i have no idea they got ma medical or not cuz my still is still "In Process" and yesterday i sent right of permanent residence fee through dhl and tomorrow monday they will deliver

simple procedure defined by Visa Offices, send an email to CHC London, they will send you the PPR-1, Dont send the passport to Islamabad Office, Isd office will send you back your passport without visa.

Send CSE immediately and explain the situation. They will change it to PPR.

I think it is Clearly written on PPR1 that plz contect us if u r living out side of pakistan we will send u another PPR form.

Just reply the mail and tell them your position.
