No, it won't be a problem. Medicals can be done anywhere.

What is done in physical examination? Do they check the complete body for any marks?

Blood test. Urine test. Chest x-ray. Vision. Physical appearance. They also ask you about your lifestyle, like your daily activities, smoking and other such things.

Yes, in some places they do check your whole body.

They just do basic check up like blood, urine, eye-sight, BP, X-Ray. If anything adverse come sin reports they ask for further investigation few days later.
I don't think city matters as long as it is from empanelled doctors/ hospitals
