Can anybody tell me if the following diseases disqualifies an appllicant:
1) Diabates
2) High Blood pressure
3) Previous Pneumonia

The purpose of Medical test as per CIC is "To protect the health and safety of Canadians, as well as reduce and prevent excessive demand on Canada’s health and social services system ". Since the conditions you mentioned are not falling in the said conditions, I do not think you should worry.

High blood pressure did entail some extra medical tests in one particular case I know, though I am not aware of specific details. Generally though, the above 3 cannot be grounds of disqualification for entering into Canada unless it causes in "excessive demands" on medical facilities in Canada.

I had high blood pressure and I had to go to a cardiologist and get an electro cardiogram to prove that my high blood pressure was not because of any defect in my heart.
