Please advise, oh wise Forum people? ;D

We sent everything -- so we thought -- to Mississauga a few days ago: all our sponsor/applicant paperwork, the FBI clearance, the proof of relationship, and the Medical form called "APPENDIX C" that the check-list called for.

The APPENDIX C form that we included w/our application had my photo on it, a stamp from the doctor, signatures, etc.

Then, yesterday, the office of the doctor (designated medical practitioner) phoned to say "wait! we gave you the wrong form! you need the form with the barcode on it! that one is supposed to be mailed in!" :'( :'(

Today, this completed form I'd never seen before, "MEDICAL REPORT - SECTION A," arrived in the mail from the doctor's office. it has a bar code, and also says on it: "For excessive demand-exempt family class clients applying from outside Canada only who present completed appendix D medical instructions."

Huh? What is it?

This "SECTION A" isn't mentioned in either my instructions for Family Class/Spouse or in the document checklist for Family Class/Spouse.

What should I do?

Should i forward it to Mississauga? Should I just wait?
 
PS: I'm an American, so my check-list and region-specific instructions were for the USA.
 
Okay, nevermind, I've answered my own question.

I was correct to bring Appendix C to the Dr's appointment.... but they were supposed to sign and return to me "Section A"/copy 2 (not the Appendix C itself).

So they did return to me and tell me to mail the wrong thing. Arg.

And I didn't realize it, because I didn't read the fine print in the checklist about "Copy 2/Section A" properly. (It doesn't mention Section A anywhere on Appendix C or in the instructions themselves. Drat.)
 
Thank you for posting this, I will be going in for my medical in a couple of weeks and have just assumed that I will fill out the section C form and have it stamped and returned. Now I know that I will get another paper. :)

I dont know what you should do with regards to sending it, maybe wait to see if they request the right document once you have a case #?

huuh? what's this Appendix D, now?

the form I brought to the doctor -- and was instructed by the region-specific instructions to bring -- was Appendix C, not D.

i'm bummed because i don't have any client ID or anything yet, so i don't know how to follow up. my own application is now in Missessauga, to be forwarded to Buffalo for review. so ... so... so they won't notice anything wrong till Buffalo. and then -- what? are they going to return it to me, as incomplete? hope not.
 
I think Appendix D is for refugees but I am not 100% on that. The immigration website is so poorly laid out it isn't even funny and so I can't even find appendix D

There is no mention of Appendix D anywhere on this site.
 
I didn't get Appendix C back.

My copy of Section A of the medical report (IMM 1017) also mentions Appendix D. I think they removed one of the appendices at some point, and Appendix D became Appendix C.

You do not need to include Appendix C, or Section A, itself. You need to include a photocopy of your copy of Section A. I'm guessing that if Mississauga doesn't return your application, Buffalo will ask for the photocopy of Section A. You may need to ask your DMP for it if it wasn't given to you.

edit: I just saw you posted another thread; I'll provide a more concise reply there.
edit again: Send the original, to be safe.
 
The checklist clearly states that the original IMM 1017 (Section A) should be sent, not a photocopy.
 
Yes DON'T send a photocopy, make sure you send the original! I just had my entire application returned to me after a month because I'd misread 'copy 2 of Section A' and thought it meant a copy of.
 
Ack! Somehow I misread the instructions and got by with just a photocopy, but yeah, you shouldn't risk it.
 
It's definitely an ORIGINAL of Appendix A- not a photocopy.

you need to send exactly that paper (form) that was given to you by doctor's office after you finished your medicals.
