Can someone clarify what medical exams option apply to me and my family before filing e-APR?

Below is the text from CIC website

When to get your exam, You have two options for when to get your medical exam. Before you choose one check the application instructions for the program you are applying for.
Option1 >>Wait for instructions once you have submitted your application
Option2 >>Get your exam before you submit your application

So, just for the sake of clarification of future visitors to thread, when does the other option apply?
Is it when, a canadain PR person wants to sponsor a partner and dependents?
